Fraction of cells remaining (FCR) and hemoglobin S (HbS) percentage both relate to blood cell percentages and may be selected by the ordering physician as an endpoint for an RBCX exchange or depletion/exchange procedure. But they aren’t the same thing.

  • FCR is the percentage of a patient’s original red blood cell volume remaining at the end of an RBCX procedure.
  • HbS% is the percentage of sickled red blood cells in the patient's blood. It can be calculated before and after an RBCX procedure.

To perform RBCX exchange or depletion/exchange on the Spectra Optia™ Apheresis System using the patient’s FCR:

 

  1. Touch FCR (%) to display the data entry pad. The message bar displays: "Enter target FCR for patient (%)".
  2. Enter the percentage on the run values screen. The system calculates the volume of replacement fluid needed to achieve the target FCR.
  3. When you are ready to perform the procedure and have received the replacement units for the exchange, enter the total replacement volume, and the system recalculates the target FCR.

To perform RBCX exchange or depletion/exchange on Spectra Optia using the patient’s HbS%:

  1. Touch FCR (%), then touch FCR on the data entry pad.
  2. The blood drop icon appears on the data entry pad and the message bar displays: “Enter starting defective RBC for patient (%).” Input this value and touch Enter.
  3. The message bar displays: “Enter target defective RBC for patient (%).” Type this value and touch Enter. The system calculates and displays the calculated target FCR.
